If you are seeking permanent residence in the United States, you will be required to submit to a medical examination to prove that there is no medical reason to deny your green card application — a serious communicable disease, for example. Not just any doctor will do, however — only a doctor licensed as a “designated civil surgeon” by the US Citizenship and Immigration Service (USCIS) will be acceptable to the USCIS. When You’re Finished With the Exam

Once you complete your medical examination, you will be given a sealed envelope containing a completed USCIS Form I-693, Report of Medical Examination and Vaccination Record. Do not open or unseal the envelope. Due to the possibility of tampering, your medical examination will be considered invalid if the seal is broken, and you will have to start the process all over again.
